Midazolam’s activities are mediated by the inhibitory neurotransmitter gamma-aminobutyric acid (GABA), which is one of the most significant inhibitory neurotransmitters in the central nervous system. Benzodiazepines cause sleepiness, skeletal muscle relaxation, and the induction of sleep, anesthesia, and forgetfulness by increasing GABA activity. Benzodiazepines bind to the benzodiazepine site of the GABA-A receptor, increasing the frequency of chloride channel opening and potentiating GABA effects. These receptors have been found in a wide range of human tissues, including the heart and skeletal muscle, although they appear to be most prevalent in the central nervous system.
